Extended Care Team Details:
Dreyer is seeking an energetic, progressive Geriatric Internal Medicine or Family Medicine BC/BE physician who genuinely loves to work with patients who reside within assisted living, skilled custodial, nursing home, and alzheimer’s facilities. Our “Extended Care Team” currently has one Physician and one Nurse Practitioner.

• Ability to work well with Dreyer’s Hospitalists when discharging patients to facilities, along with office primary care providers.

• Ability to provide coordinated care through use of EPIC EMR.
  Ideal candidate must have an incredible capacity to connect with patients and their  families to grow these life‐long relationships.
• Ability to successfully engage families in end of life discussions when applicable.
  Responsiveness to the facilities that we serve to meet their needs of patient care, whether this involves training staff or coordinating patient care.
• Eventual work, patient care/coordination will show a decreased re‐admission to the hospital.
• Desire and ability to grow this service‐line and eventually add more providers and services by working with administration within the facilities.

Pulmonology Department Details:

• MRI, 64‐Slice CT Scanner, Ambulatory Surgery Center at Highland site, along with cardiac and surgical subspecialists.
• 1 in 4 call on weekdays and weekends.
• Main Pulmonary office located at Dreyer’s Highland site, with a secondary location at our brand
• new Rush‐Copley Campus location (to be opened in October 2011).
• Opportunity for partnership in sleep medicine clinic. Sleep medicine involves about 20% of thepractice. The new partner will have an opportunity to buy into the sleep center (after shareholder status is approved). A sleep‐boarded specialist will also have the opportunity to beMedical Director of the lab.

Competitive Compensation and Generous Benefits Package:

• Competitive 2‐year income guarantee with potential to exceed.
• 5 weeks paid time off for vacation or CME; 6 weeks at 2 years.
